New Patient Registration

For a more efficient clinic visit, it is helpful to do the following:

1. Fill out the New Patient Registration which will create your personal medical record in the Patient Portal.
2. Go to “Request Appointment” and make an appointment on the Patient Portal.
3. Fill out your paperwork (located in “Questionnaires” in the Patient Portal) prior to your appointment
4. Add a credit card (located in “Invoices & Payment” in the Patient Portal) to have more time to discuss your clinical issues. You will not be charged until the end of your appointment.
5. Bring all your medications and supplements for review during this appointment.
